Payment is expected at the time of services
All payments are required at the time services are rendered unless arrangements have been made in advance. This includes all applicable co-payments and co-insurance for participating insurance companies. We accept cash, in-state personal checks, credit/debit cards, and CareCredit.
For our self pay patients, we require 50% of the operative fee be paid prior to scheduling the surgical procedure. This is for general surgery services, see below for cosmetic services.
Insurance
Your insurance card and photo ID must be presented at your first visit. Our office will submit charges for noncosmetic treatments to the patient’s insurance company. However, the patient is responsible for paying any and all medical expenses incurred. You are expected to pay your deductible and co-payments at the time of service.
Prior to your appointment we attempt to verify that specific medical/surgical procedures are covered by the patient’s insurance company. Occasionally, even though coverage was verified before the surgical services were provided, the insurance company denies the claim. If your claim is denied, you will be personally and fully responsible for any remaining balance.
Cosmetic Services
All cosmetic fees must be paid prior to service. The cost of cosmetic services will be reviewed and agreed upon prior to starting the procedure. Cosmetic consultation appointments are $50 and this fee is applied to any products or services purchased.
Healthcare Financing with CareCredit
We do accept CareCredit, a healthcare credit card that can be used to pay for out-of-pocket expenses not covered by medical insurance and offers special financing options that you may not be able to get with traditional credit cards. Additional Charges
Returned check: There will be a $25.00 service charge on any returned checks.
FMLA or Disability Paperwork: There are no charges for completion of FMLA or disability paperwork when associated with operative procedures.
